EPIC Benefits+ 

 

If you do not want to make changes to your EPIC Benefits+ coverage, no action is necessary.

 

There are no changes to EPIC Benefits+ benefits planned for 2013.

 

No Open Enrollment: There will be no special/open enrollment this year. You cannot add vision coverage for 2013 if you do not already have it.

 

Allowed Coverage Changes:

  • You may cancel the vision portion of your coverage effective December 31, 2012. If you choose this option, you will not be able to add vision back to your plan at a later date.
  • You may remove dependents from your plan effective January 1, 2013.
  • You may cancel your plan to be effective January 1, 2013.
  • All requests for changes must be made via an EPIC Benefits+ Enrollment Form and be submitted to Human Resources between October 8 and November 2, 2012.
