
Adult Children (age 19 to 26) Benefit Information Sheet
Effective January 1, 2012, your adult children may be covered through the end of the month in which they turn age 26 under the health, supplemental Delta Dental, EPIC Benefits+, and VSP Vision Care insurance plans.

Annual Opportunity to Change your Adult Child(ren)'s Health Coverage
The annual It's Your Choice enrollment period (October 8-November 2, 2012) gives employees currently enrolled in health insurance an opportunity to enroll or cancel their adult child's health insurance coverage as of January 1 of the following calendar year.
Health Insurance: You may enroll or cancel your adult child’s health insurance effective January 1, 2013 by submitting a Health Insurance Application/Change Form to Human Resources between October 8 and November 2, 2012.
EPIC Benefits+ Insurance: You may cancel your adult child’s EPIC Benefits+ coverage effective January 1, 2013 by submitting an EPIC Enrollment Form to Human Resources between October 8 and November 2, 2012. There is not an open enrollment this year for EPIC Benefits+ so dependents cannot be added to your plan unless they experience a qualifying event.
VSP Vision Care Insurance: You may enroll or cancel your adult child’s VSP Vision insurance effective January 1, 2013 by contacting VSP directly at 1-800-400-4569 between October 8 and November 2, 2012.
Supplemental Delta Dental Insurance: You may cancel your adult child’s Delta Dental coverage effective January 1, 2013 by submitting a Delta Dental Enrollment/Change Form to Human Resources by December 1, 2012. There is not an open enrollment this year for Delta Dental so dependents cannot be added to your plan unless they experience a qualifying event or you add them as a late applicant (waiting period applies to most benefits).
Note: If your adult child is currently covered under your insurance(s) an application is not necessary to continue coverage.
Mid-year enrollment opportunities may also exist if your adult child experiences a qualifying event such as a loss of insurance coverage from their employer. You will not be able to add your adult child onto your insurance(s) mid-year unless your child experiences a qualifying event. Please note: Change in student status is no longer be considered a qualifying event for enrollment or cancellation.
