A Community Health Needs Assessment (CHNA) looks at the health of a community by using data and collecting community input. CHNAs examine community health from a big-picture view and consider risk factors, quality of life, mortality, morbidity, access to health care and more. They assist in establishing priorities for community health and developing and implementing community health programming.
To assess the health needs of Dane County, four area hospitals - Meriter Hospital, St. Mary's Hospital, Stoughton Hospital and UW Hospital and Clinics - joined with Public Health Madison and Dane County to form a collaborative group known as Healthy Dane.
Our website, HealthyDane.org, provides continuous updates on numerous health indicators, including social determinants of health. It is an example of the trend away from static written reports and toward an interactive and dynamic tool available to all community members.
