Take Control of Your Diabetes

The UW Health providers at Sun Prairie would like you to attend an innovative class taught by UW Health Certified Diabetes Educators for patients with diabetes. This session will be led by a registered dietitian and registered nurse providing you with individualized education from an American Diabetes Association Recognized Program. | Diabetes Management

 

Goals

  1. Discuss ways to improve your blood sugar that will fit into your lifestyle
  2. Understand why the American Diabetes Association recommends your A1C be lower than 7percent
  3. Make a plan for you to take back control of your diabetes

Fees are billed to your insurance and are subject to the conditions of your plan.

Please call the Sun Prairie clinic at (608) 837-2206 to enroll in the class of your choice as soon as possible as space is limited.
