UW Health's bariatric surgery program offers an educational seminar to new patients. You can attend the seminar in person or access all of the seminar information and registration materials online.

Upon registering for the educational seminar, you will be sent a Complete Health Questionnaire. We ask that you please completely fill out and bring with you to seminar. Also bring front and back copies of your insurance card with you.
