Referring Your Patients to the Pain Treatment and Research Center
Our triage staff will contact you with the patient's appointment date once the visit is scheduled. Patients are required to complete an orientation session before their first visit. Our staffers will update you when the patient's initial assessment is complete, as treatment progresses and when the patient is stable for discharge. We encourage you to contact the clinic staff to discuss issues that arise during treatment or follow-up. How To Refer Patients
To order a Pain or Headache Clinic visit, please download and complete one of the following: Fax the form to (608) 263-0135. Please include copies of pertinent medical records if referring from outside UW Health. Our clinic referral criteria are listed on the second page of the form.
