Children Matter Award Nomination Now Accepted Year Round

The Children Matter Award program, sponsored by American Family Children's Hospital in Madison, Wisconsin, recognizes any employee or volunteer at UW Health who has demonstrated the principles of compassionate care.

 

Patients, their families, UW Health staff and volunteers may make nominations. The Children Matter Award Committee, comprised of individuals from across American Family Children's Hospital departments, will evaluate all nominations.

 

 

Using the Children Matter value statment (CHILD acronym), explain why the candidate should be considered for a Children Matter Award. Please submit one candidate per nomination form.

 

Please note, you do not need all of the candidate's information in order to submit a nomination. A name and department are sufficient. All nominations are kept confidential.
