Children Matter Award Example Nomination
The Children Matter Award sponsored by American Family Children's Hospital in Madison, Wisconsin, recognizes staff and volunteers who embody the values of compassionate care.
Patients, their families, UW Health staff and volunteers may submit nomiations through the web form.
As part of the nomination process, individuals are asked to explain why the candidate should be considered and how he or she demonstrates the principles outlined in the Value Statement.
Following is an example of the narrative portion of the nomination.
Value Statement

Dedicated to the overall care of hospitalized children and their families.
Example Submission
"Our nurse was compassionate enough to go the extra mile to get our daughter to MRI.
They wanted to push it until 5pm and she hadn't eaten since 12:30am. But the nurse fought and got her in for noon instead.
She was helpful in every way. She did everything she could to make and keep my daughter comfortable. She was involved with the attending physician to get things going more smoothly with the MRI. She listened to all of our concerns and frustrations.
That evening when we received more difficult news, she listened to the sound in my voice when she asked me if I was okay. I said yes, but she knew I wasn't and really care and wished she could do something to help.
She was the most dedicated nurse we got the pleasure to work with. She always went the extra mile no matter how small the request. It's hard to find people like that.
She is a great nurse."
