nteral Team Nurse


Position Summary: The Enteral Team Nurse is responsible for the coordination of patients' home nutrition care. Responsibilities include assessment, ongoing monitoring and preauthorization of services for patients receiving home enteral nutrition. This position works closely with multiple disciplines, including physicians, social workers, registered dieticians and insurance case managers to ensure patient-focused medical goals and objectives are being met and proper documentation is obtained. This position required on-call activities.


Education/Experience: Must be a Licensed Practical or Registered Nurse with a current, unrestricted license and a minimum of two years of recent relevant experience. Home health and enteral therapy experience with a focus on compliance with commercial insurance or government payer reimbursement preferred. Excellent organizational skills and ability to coordinate services within a multidisciplinary team environment required. Must have a valid driver's license.


Pay will be commensurate on licensure.

Branch Admissions Coordinator


Position Summary: The Admissions Coordinator is responsible for coordinating the referral/admission process for patients receiving enteral and parenteral therapy. Facilitates all nursing, pharmacy, reimbursement and operations activities related to the admissions process. Provides extensive, ongoing interaction with patients, physicians, referral sources and community agencies. Orients and acts as preceptor for new staff.


Education/Experience: Nursing registration in state employed. BSN preferred. 2 years of acute care, home health or infusion nursing experience. Discharge planning, continuing care, or home infusion therapy experience is preferred. Excellent organizational, interpersonal, communication and customer service skills.
