We are currently accepting applications for the following positions:


Transitional Care Coordinator


Position Summary: Coordinates home infusion services between the hospital, outpatient clinic, other alternate care sites and Chartwell Midwest Wisconsin (CMW). Facilitates infusion discharges through collaboration with facility staff and patient/caregiver training. Identifies opportunities for the CMW care plan, and/or alternative plans of care, as needed. Acts a resource to UWHC coordinated care and medical staff regarding CMW services and specific patient problem resolution. Provides CMW office-based staff with appropriate information to facilitate infusion discharge planning. This position requires on-call activities.


Education/experience: Graduate of a registered nursing program with a current, unrestricted license, BSN preferred. At least two years of recent relevant nursing practice. Hospital discharge planning, home health case management or home infusion experience preferred. Effective communication skills and the ability to collaborate with a multidisciplinary team to organize, prioritize and coordinate patient care required.


Pharmacy Technician I - Associate


Position Summary: Assists with the compounding of intravenous solutions and injectable medications according to the principles of strict aseptic technique and assures compliance to all sterile product area quality control policies and procedures and professional standards. Assists with technical and clerical duties involved in the day-to-day operations of the pharmacy as allowed by law and/or regulation.


Education/experience: High school diploma or equivalent. Minimum of one year experience as a pharmacy technician with training/experience in alternate site/home health pharmacy practice and the preparation of sterile product preferred. Completion of a pharmacy technician training program and/or certification by the Pharmacy Technician Certification Board highly desirable.
