Clinical Management Services
Chartwell is respected for its expertise in Clinical Management and Care Coordination. A person receiving home infusion therapy is assigned to a clinical management team who provides advanced Clinical Pharmacy and Nursing Management. The team is comprised of a Clinical Pharmacist, Nurse Coordinator, Insurance Verification Coordinator and Patient Services Coordinator. This team provides meticulous attention to each individual person's care needs. Extensive follow-up occurs with the individual, physician and home health nursing agency.
