Unity Health Insurance, a UW Health-affiliated company, was formed in 1994 to provide community-based managed health care to more than 90,000 members throughout 20 counties in southwestern and south central Wisconsin.Unity provides HMO, Point-of-Service (POS), and Preferred Provider Organization (PPO) plans to individuals, families, businesses and State of Wisconsin employees. Unity also offers HSA-qualified plan designs and Medicare Supplement coverage.
Unity's parent company is University Health Care, Inc. UHC is comprised of University of Wisconsin Hospital and Clinics (UWHC) and University of Wisconsin Medical Foundation (UWMF). Unity Health Insurance is located in Sauk City.
